Clopidogrel: 18.75mg (¼ tab) once daily — given at night with PM furosemide, with food
Rivaroxaban: 2.5mg once daily — given at night with PM furosemide
Before next echo: Gabapentin 100mg — night before & morning of

Watch for: hindlimb weakness/paralysis (emergency), bleeding signs (rivaroxaban), resting rate ≥35 bpm or a rising trend.
